Narrative Report
· Changing utilization of medical procedures and resources in diabetes management is the point of intervention for quality improvement in health care in order to decrease the risk of diabetic complications. Traditional quality indicators fail to integrate the process and outcome aspects of patient care. Acceptance of the process-outcome parameters assumes that providers, intended users, acknowledge the relevance of quality measures and recommendations. Therefore, recommendations or targets need to be not only clearly defined but also substantiated by compelling evidence. Acceptance of quality parameters and targets can be evaluated be surveying a representative group of physicians. My researches focused on developing validated process-outcome parameters for improving the quality of diabetes care among primary care physicians. It also focused on developing versatile computer programs to facilitate auditing of these parameters and connecting satellite primary care clinics with a supervising diabetes centers.
· Insulin pump therapy is a thoughtful alternative to the conventional intensive insulin regimen in type 1 diabetic patients. My researches suggested that the improved diabetes control, with CSII, can not be solely related to the pump usage; as additional factors were concurrently implemented such as the increased motivation of the selected patients, the intensified follow up schedule, the frequent blood glucose monitoring and patient’s incorporation in supervised diet and exercise programs. The dilemma of follow up and the lacking of specified centers are additional factors. I’m working on developing new electronic devices that may aid pump users as well as diabetologists planning to implement CSII therapy and I developed a computer algorithm to help programming the insulin infusion pumps with multiple basal rate capabilities.
· Obesity, hypertension and impaired glucose tolerance are frequently concomitant as components of the metabolic syndrome together with dyslipidemia, insulin resistance and hyperuricemia. My researches suggested that not only the extent of weight loss is the determent of the magnitude of blood pressure control in obese patients with metabolic syndrome but also the pattern of body composition, especially fat/lean ratio, is much more important. Conservation of lean body weight by exercising and by increasing protein portion in the diet regimen must be of prime concern in diet planning for obese patients. Improvement of fat/lean ratio with this regimen could help in lowering control of blood pressure, blood glucose and serum cholesterol.
· Lifestyle modification is an integral part of the common recommendation for obese type 2 diabetic patients with insulin resistance syndrome. Previous researches demonstrated that weight reduction through diet and exercise improves insulin sensitivity. My research is focused on testing the effect of life style modification on the endothelial function and vascular reactivity in obese subjects with insulin resistance syndrome. I’m also trying to find any relation between the change in endothelial function and the changes in body composition, physical fitness, insulin sensitivity and beta cell functions after life style modification.
· There is veritable evidence denoting an increase in the concentration of oxidized products in plasma of human diabetic patients. It was also suggested that both oxygen free radical reaction and non-enzymatic glycation might play an important role in the development of diabetic complications. My researches suggested a possible relevance of lipid peroxidation to the development or evolution of diabetic neuropathy. In a multi-center clinical trial we are planning to test the hypothesis that increased oxidative stress is one of the most important risk factors in the development of diabetes neuropathy in type 1 diabetic patients and that intervention with antioxidants may retard or prevent its development.
