Print a copy of your order form (last step of online
ordering process) and place with samples for proper identification. |

Sample drop off at the Assay Lab - room 430. Place
samples with order form in designated freezer or speak to James Lolley or
Hui Li for assistance. |

*Samples placed in freezer
WITHOUT an order form will NOT be processed. |

Your test results will be emailed to you in an Excel
spreadsheet. |
| |
Please pick up any remaining sample material within
2 weeks after receiving your results. |
